INSTRUCTIONS FOR FILING A VOUCHER ON LINE
(Be sure to Print a copy of this page before going to the Voucher Site using the link at the bottom of the page.)


+ When the Web page comes up on the screen, take the mouse pointer to the left side (CS3) and click.

+ Take pointer to the right side of the screen where it says Returning Users. Enter SS#, Password (PIN), last name, 5 digit zip and Birthdate. This is entered as mm/dd/yyyy, year is entered as all four digits. Click next.

+ You will then be prompted to enter Password (PIN) twice more. Click next each time

+ Answer questions on main menu by clicking yes or no. Then click next.

+ Click on claim voucher inquiry (here you can check on your voucher status) then click submission and next.

+ Click I want to submit information selected voucher then click next.

+ Read the unemployment fraud disclaimer, if you agree click on I agree.

+ Answer questions on voucher regarding pay, work search etc. when completed click next.

+ Check review screen to see if all information looks correct, if so submit.


Some things to Remember

+ Be certain that when you enter Birthdate that you put entire year, for example if it says 02 on the voucher you receive in the mail you have to put 2002 on the computer voucher.

+ If you enter a wrong answer FIVE(5) times, such as 02 instead of 2002, you will get a STOP SIGN and have to Contact the local office to reset your Password (PIN).

+ If you are not fully registered and do not have a waiver and say no to the question "Do you want to enter more information now?", you may get a Stop Sign and have to contact the local office.
